Level of Care Assessment

Choosing the right kind of senior care can feel overwhelming. This short assessment gives you a plain-language starting point.

Most families come to us with the same question: how do I even know what my mom or dad actually needs? The options — in-home care, independent living, assisted living, memory care, skilled nursing — all sound different, cost differently, and every community defines their "levels of care" a little differently. It's a lot to sort through while you're also worrying about a parent or spouse.

We put this short assessment together to make that first step easier. It's not a medical evaluation, and it doesn't ask for anything sensitive. Just a few questions about daily life — how they're doing with bathing, meals, memory, medications, and getting around — and it gives you a plain-language sense of what level of care may fit.

You'll see your results right away, and we'll send you a copy to keep. From there, you can call us with questions — or take the results to any provider you're already talking to.
